Registration
Pre-registration

Registration:

All new patients wishing to register must fill out a Health Questionnaire.

Before registration is processed, it is the Bexley Medical Group’s policy that all new patients attend a health screening carried out by our Health Care Assistant; the screening will take approximately 10 minutes.

No registrations will be completed unless you have attended the Health Screening.

You must bring your completed health questionnaire and a urine sample with you to your appointment; the receptionist will provide you with a sample bottle when you make your appointment.

All new patients wishing to register need to bring photo ID, which will be photocopied, this could be one of the following:

  • Passport
  • Driving Licence (photo ID only will be accepted)
  • Freedom Pass

New patients also need two proofs of address:

  • Council tax statement
  • Utility bill

Patients from outside the UK will be asked to provide a Passport, valid Visa, Tenancy Agreement or Permanent Settlement document.
